Recertification Appeal Procedure
If a recertification application is denied and the applicant decides to appeal the decision, the appeal must be submitted in writing to the Appeals Committee within 30 days of receipt of the letter of denial, and sent to:

All appeals must be in writing and must contain the rationale for the appeal and all supporting documentation. The Appeals Committee will endeavor to render an opinion within a period of approximately 30 days. The decision by the Appeals Committee will be final.
