Certification Agreement Form

The Certification Application Form and all supporting documentation should be mailed by the applicant to the Board of Certification c/o SMT. The Certification Agreement Form (see below) must also be signed and dated by the applicant. If the applicant is approved for certification, this Agreement will be counter-signed by an appropriate representative of the Board of Certification and returned to the certified practitioner. Recertification will require signing of a similar Agreement.
